Medication safety

Administers medicines using the rights of administration, independent double-checks for high-risk drugs, and reports near-misses and errors openly.

Patient-centred care & dignity

Plans care around the patient's needs, preferences, language and culture; keeps the patient and family informed and preserves dignity, especially with vulnerable patients.

Infection prevention & control

Applies standard and transmission-based precautions consistently, and challenges lapses in hand hygiene, aseptic technique and isolation by colleagues.

Documentation & handover

Records care accurately, contemporaneously and legibly, and hands over patients so that the next shift can act safely without re-asking.

Manages a full patient allocation with competing demands, delegates appropriately to enrolled and auxiliary nurses, and asks for help before care becomes unsafe.

Works within SANC scope for their category, refuses unsafe instructions respectfully, and takes personal accountability for actions and omissions.

Diagnostic reasoning

Builds a differential from history and examination, orders investigations that change management, and revises the working diagnosis when findings do not fit.

Recognising deterioration & escalation

Identifies the sick patient early, initiates resuscitation and treatment within competence, and calls for senior or specialist help without delay.

Communication & shared decision-making

Explains diagnosis, risks and options in plain language, obtains informed consent, breaks bad news with care, and respects a patient's right to decline.

Clinical governance & error disclosure

Participates in audit, morbidity and mortality review and incident reporting; discloses errors to patients honestly and changes practice as a result.

Leading the clinical team

Runs ward rounds, resuscitations or theatre lists with clear roles, supervises juniors safely, and handles conflict with nursing and other specialties constructively.
